The hospital address is 355 N Main St., Kanab, UT 84741. Enter through the main entrance of the hospital; KCH Medical Practice is located just to the right of the main lobby.

New Patients and Returning Patients

KCH Medical Practice welcomes new and returning patients. If you previously received care through Kanab Family Medicine or Revere Health, your care remains our priority as the clinic continues under Kane County Hospital.

For records or billing before April 1, patients should continue using the Revere Health portals.

For records or billing after April 1, care is managed through KCH portals.
